A successful rhinoplasty aims to achieve a nose aesthetic that is formally compatible with the person’s face, as well as creating a breathing, functioning nose…
Although beauty is a relative concept, aesthetics is a set of criteria that can be evaluated within the framework of certain norms and forms, that can be measured and that we agree on.
In addition to being formally compatible with aesthetic forms, the beauty of something is also related to its usefulness in terms of content, because beauty arises from the consistency of form and essence.
While rhinoplasty is evaluated within these measurable beauty boundaries, it has also received various updates from developing surgical techniques and changing aesthetic approaches.
For example, those who have rhinoplasty usually want “it not to be obvious that it is aesthetic”. The way to do this is to make noses that are as natural as possible, compatible with the person’s facial features and skin structure. However, today it is possible to come across individuals who want to have noses that stand out aesthetically.
Rhinoplasty patients are people who have given all their attention to their noses and have examined the defects in their noses many times over the years. Therefore, their first thought is to have a nose that looks very beautiful and even attracts attention with its beauty, instead of a nose that is compatible with their facial features, which is a very understandable state of mind.
However, rhinoplasty is much more than just the aesthetic reshaping of the nose…
Rhinoplasty, derived from the Latin word Rhino meaning nose, is the name of aesthetic nose surgery in the aesthetic/plastic/reconstructive surgery literature and its widespread use worldwide.
Its main purpose is to reshape the appearance of the nose. Especially from the patient’s perspective, rhinoplasty is largely related to the appearance of the nose, but from the surgeon’s perspective, aesthetic adjustment is the most important part of rhinoplasty, but it is not the only subject.
Rhinoplasty, or rhinoplasty as it is known in the literature, is a surgical procedure that can be performed with open or closed techniques to eliminate visual complaints about the nasal dorsum, nasal tip, nasal length and size. The nose is reshaped in a way that is compatible with the facial features, taking into account the patient’s expectations.
Having a nose that is smooth and compatible with the anatomical structure of the face allows us to achieve a facial expression close to the measure of beauty known as the golden ratio, so the nose is one of the important criteria in the aesthetic appearance of the face.
For this reason, among cosmetic operations, rhinoplasty surgery is among the most frequently performed aesthetic operations in the world.
The nose is an extremely complex structure consisting of bones, cartilage and soft tissues that activate our sense of smell, breathe, move and are also affected by heat. Rhinoplasty covers aesthetic and functional surgical procedures that are performed by considering all these parameters and whose aesthetic and effective dimensions are equally important.

Surgical Techniques Used in Rhinoplasty Surgeries
Open rhinoplasty and closed rhinoplasty are the two basic techniques we use in rhinoplasty surgeries.
Taking into account the nasal anatomy, skin structure and other aesthetic parameters, the surgeon determines which of the open or closed rhinoplasty surgeries will provide the closest result to the patient’s request.
What is Open Rhinoplasty and How is it Performed?
Seventy percent of rhinoplasty surgeries performed all over the world are performed with the open technique. The reason for this is that closed rhinoplasty is not the appropriate method for every nose. Open rhinoplasty expands the possibilities of the operation as it allows the opportunity to intervene by seeing the nasal anatomy and provides the opportunity to remove some soft tissue in thick-skinned noses.
Open rhinoplasty is preferred especially in thick-skinned nose surgeries and in large and high-arched noses. Nevertheless, the surgical method is a controversial issue among doctors.
Open rhinoplasty, which was the only valid method in rhinoplasty surgery until recently, is the classic rhinoplasty method. It is performed under general anesthesia by lifting the skin with incisions opened under the nose.
With open rhinoplasty surgery, the nasal bone and cartilage tissues are reshaped, and functional problems that cause respiratory problems such as deviation or nasal flesh, which we call intranasal bone and cartilage curvature, are eliminated.
Open rhinoplasty surgeries are applied to eliminate a wide variety of problems such as nasal tip aesthetics, nasal hump aesthetics, nose reduction, nose shortening, nose tip lift, and nasal curvature.
The important thing is to choose the right technique for the right nose and to perform the operation in the best possible way.
Open Rhinoplasty Advantages:
Open rhinoplasty allows you to reconstruct the nasal anatomy just like a Lego. When we want to reconstruct the bones, cartilage and connective tissues, the open method is our choice.
Our area of control is more comfortable in the open rhinoplasty method compared to the closed rhinoplasty technique. Since it is easy to work in open rhinoplasty, we can give the nose the shape we want and make a functionally flawless nose.
The scar after open rhinoplasty is too weak to be seen with the naked eye. This small incision under the nose is only noticeable when looked at very closely. In this respect, it is not very different from the closed method.
Open Rhinoplasty Disadvantages:
Tissue trauma is more compared to the closed method. Edema, bruising and swelling occur more frequently after open rhinoplasty than in the closed method. Swelling after nose surgery is a very normal result. Its severity varies in each case depending on the skin structure and the content of the operation performed.
What is Closed Rhinoplasty, How is it Performed?
The closed rhinoplasty technique is performed without making an incision. It is a closed aesthetic nose surgery technique performed with small incisions opened from the nostrils in noses that can be corrected with minor interventions.
In noses that do not require intervention on the tip of the nose, that look beautiful when viewed from the front but appear high or arched from the profile, it is possible to correct the bridge of the nose with closed rhinoplasty.
Advantages of Closed Rhinoplasty:
There is less bruising and swelling after closed rhinoplasty. The recovery process is shorter than open rhinoplasty.
Disadvantages of Closed Rhinoplasty:
Closed rhinoplasty is not a suitable method for everyone, open rhinoplasty is the valid surgical technique in cases where the tip of the nose needs to be reconstructed.
What we can do with closed rhinoplasty surgery, which we perform by removing parts from the nasal bone, is technically limited.
Rhinoplasty Approach Methods
In rhinoplasty surgeries, we can talk about 4 techniques that we use in both open and closed methods to shape the nasal bone and soft tissues:
Ultrasonic Rhinoplasty (Piezo Technique)
Piezo method nose aesthetics is used in both open and closed rhinoplasty. This technique, which we use to shape the nasal bone, reduces surgical trauma in rhinoplasty.
The piezo device used in ultrasonic rhinoplasty does not damage soft tissues, veins and nerves while shaping the nasal bones during surgery.
With piezo rhinoplasty, we can obtain prismatic curves at the tip of the nose in thin-skinned noses that will reflect light refractions. In thick-skinned noses, we can thin the soft tissue to a certain extent thanks to nose tip surgery.
Conventional Rhinoplasty (Classic Rhinoplasty)
Conventional aesthetic nose surgery, which we call classical rhinoplasty, is the traditional method used in both open and closed rhinoplasty.
Micro-Turn Rhinoplasty
It is a method of shaping the nasal bones without breaking them and removing their excess by using low-frequency and high-frequency diamond and carbide drill tips during surgery. It gives very good results in selected cases. It is not possible to apply it to every nose.
Combined Rhinoplasty
Combined rhinoplasty is a term we use to express two different procedures in rhinoplasty surgeries.
The surgical approach we apply by combining ultrasonic, micro tour and classical methods is called combined rhinoplasty. Both piezo and tour are devices that can be used at the same time. The combination of experience and technology allows planning personalized approaches instead of operating with the same technique for every patient. Thus, we can design procedures that will provide maximum benefit.
Types of Rhinoplasty Surgery
Types of rhinoplasty surgeries that vary according to the structure of the nose, the area where the procedure will be applied, the number of surgeries and the procedure method.
Septoplasty (Deviation Surgery)
Septoplasty is a nose surgery performed to eliminate the deformities in the nose, which we call deviation. The external appearance of the nose is not interfered with in deviation surgeries.
Septum is the name of the structure located inside the nose and symmetrically dividing the nose into two, consisting partly of bone and partly of cartilage. The curvature of this structure due to congenital or traumas over time is called septum deviation, and the operation performed to correct this curvature is called septoplasty.
There is no change in the external appearance after deviation/septoplasty. It is a surgical intervention performed entirely to improve function. There is no bruising or swelling.
Septorhinoplasty
In septorhinoplasty, we aim for the nose, which is corrected not only functionally but also formally, to be both aesthetically suitable for the face and to function at an optimal level.
Tipplasty (Nose Tip Aesthetics)
Tipplasty is only nose tip surgery. It includes operations on the nose tip to eliminate problems such as drooping nose tip and deviated nose tip.
Tipplasty has recently become widely applied in patients who want to get rid of defects on the tip of the nose but are satisfied with the bridge of the nose.
Since nasal tip aesthetics requires the reshaping of the cartilage structure that forms the tip of the nose, the open tipplasty method is usually used. The drooping nasal tip is lifted, curvatures are corrected, the nasal tip is reduced if it is large, asymmetry problems are eliminated, and the lip and nasal tip distance is brought closer to the aesthetic ratio.
Functional Endoscopic Sinus Surgery/FESS Sinusitis Surgery
Functional Endoscopic Sinus Surgery (FESS) is another type of nasal surgery performed on the sinuses, which are a very serious anatomical point on our face, using small and delicate surgical instruments while the images obtained with endoscopic systems are viewed on the monitor.
Revision Rhinoplasty
The second nasal surgery performed in cases where patients who have previously undergone rhinoplasty do not like the aesthetic appearance and/or are disturbed by the functional deficiencies of the nose, or due to both complaints being present at the same time, requiring the nasal surgery to be repeated is called revision rhinoplasty.
Non-surgical Rhinoplasty
Non-surgical rhinoplasty, also known as injection rhinoplasty, is not a surgical procedure. It is used to correct the deformities of the nose. Today, it is a method increasingly preferred by individuals who are afraid of surgery. It is used to eliminate problems in the middle line of the nose. It provides a significant improvement and correction in the profile image.
Ethnic Rhinoplasty
The shape of the nose and facial structure vary according to climate and geographical conditions. Ethnic rhinoplasty is an aesthetic approach that aims to make the small noses of Far Eastern or African American individuals with weak cartilage structure look prominent and larger in a way that is compatible with their facial features.
With ethnic rhinoplasty, wide nostrils are reduced. The shallow nasal root and nasal dorsum are supported with cartilage grafts. The low nasal tip is at an optimum level.
e is brought. While doing all these, it is considered to obtain an image that is compatible with the chin and forehead area.
Since the basic problem is the weak cartilage support, the person’s own cartilage tissue is usually used in order to obtain strong cartilage supports. Rarely, cadaver cartilages can also be used. It is possible to obtain satisfactory results in ethnic rhinoplasty surgeries that require serious experience and sensitivity.
Rhinoplasty Types According to Nose Problem
Crooked Nose Aesthetics
A crooked nose is a deformed nose shape that creates s-shaped curvatures that shift to the right or left or both when viewed from the front.
Correcting it is quite a laborious and difficult job. We try to bring the nose to the midline by creating the desired fractures with a fine and meticulous work and using a number of grafts.
The method we usually prefer in these types of noses is the septorhinoplasty surgical procedure.
Arched Nose Aesthetics
The nasal hump deforms the profile image. The severity of the aesthetic effect caused by this deformity varies depending on the height of the hump. The eyes, chin tip and lips are overshadowed by a large protrusion and look different than they are.
The nasal hump, which usually appears later during adolescence, can grow gradually and the most disturbing thing among nasal defects is the nasal hump.
We can correct the nasal hump with open rhinoplasty or closed rhinoplasty surgery. The profile image changes completely after the nose surgery performed with the appropriate technique for the person.
By removing the hump on the bridge of the nose and repositioning the tip of the nose, we achieve an aesthetically flawless line from between the eyebrows to the tip of the chin.
Broken Nose Surgery
These are the types of noses where the functional and formal capabilities of the nose are weakened or completely lost as a result of traumas such as physical intervention, impact, traffic accident.
Nostril Problems
The correction of problems such as notches seen on the edges of the nostrils and the nostrils being clearly visible when viewed from the front, very obvious nostril asymmetries due to the shifting of the nasal tip cartilage, and very wide and/or wide nose wings are evaluated within the scope of nostril aesthetic problems.
Breathless Nose
Swelling of the conchae inside the nose due to nasal bone and/or cartilage curvatures, the presence of polyps, which are pathological findings, and valve problems that occur together with the squeezed nose appearance are certain problems that cause difficulty breathing through the nose.
These problems are eliminated during the rhinoplasty procedure with a correct preoperative process and planning.
Low Nose Tip (Tipplasty)
When viewed from the side, the height from the base of the nose to the tip of the nose should make a certain angle with the lips. The narrowing of this angle becomes apparent when the tip of the nose gets closer to the lip. It is easier to clearly observe the narrowing of this angle in patients with long noses.
With tipplasty surgery, it is possible to significantly correct both visual and functional improvement by bringing the fallen nose tip to its proper anatomical position.
Tense Nose
It is a condition characterized by the nose rising in a way that creates a hump in the cartilage and bone area and taking a shape that makes the nose tip look long and low.
In short, it can be defined as a letter that is too big for its envelope. In most patients, there is a hook-like nasal dorsum when viewed from the side.
By correctly identifying the problem and making the nasal cartilage and bone structure suitable for the skin in a way that is suitable for the anatomy, we can achieve the desired appearance. In other words, we can say that tense nose surgery is a way of adapting the dress to the body.
Wide Nose
If the nose is wider than it should be, a deviation from the golden ratio, which is the decisive and universal criterion for the face to be aesthetically attractive and beautiful, may occur.
For example, if the position of the nose wings exceeds the distance between the two eyes, which is frequently seen in ethnic noses and when the nose height is short or in traumatic noses, the nose wings spread and thicken. This causes very large nostrils and the golden ratio is disrupted.
Similarly, if the problem is correctly identified and the correct techniques and aesthetic approaches are used, we can achieve the desired appearance on the nose wings. Sometimes the nose width can only occur due to the widening of the nasal bones, and this situation can be resolved with sidewall aesthetics.
Thin Skin Rhinoplasty
Sometimes the skin of the nose becomes so thin that the entire bone and cartilage skeleton underneath is exposed. For this reason, it is important to work carefully in thin skin rhinoplasty surgeries. It should be performed with maximum precision and correct grafting methods, without forgetting that small defects will appear very large.
Thick Skin Rhinoplasty
Since the patient cannot change the skin structure he has, he must first understand the current situation, so that at this point the doctor can
It increases the relevance. It is extremely important for the doctor and the patient to understand each other in terms of realistic expectations. Otherwise, disappointments may occur.
It is quite possible to have a thick-skinned but aesthetically beautiful nose.
In patients with thick skin, it is possible to thin the skin as much as possible, reduce the subcutaneous tissue to physiological limits and obtain a distinct type definition by using the right cartilage graft techniques.
In some patients, it may be necessary to support the post-operative recovery process with subcutaneous injections.
Male Rhinoplasty
It is thought that rhinoplasty is not that important for men, but many men who are not satisfied with their nose but are hesitant to have plastic surgery would want to have their noses done if they were sure that the result would be natural…
The number of men who have rhinoplasty is still much lower than women, but we can say that it is increasing every day.
Men who usually have very long, very large noses, clearly deformed or extremely arched noses apply to the doctor for rhinoplasty. Minor flaws are aesthetically tolerated within male facial features, but men who dream of a perfect nose can also benefit from all the possibilities of open or closed rhinoplasty.
The key point of rhinoplasty in male patients is to maintain the masculine image, and when performing rhinoplasty in men, care should be taken to ensure that the angle between the lip and the nose does not exceed 95 degrees.
Female Rhinoplasty
Rhinoplasty in women has many differences. While one of the main goals is to bring the face closer to the golden ratio, the patient’s wishes should be taken into consideration, provided that they remain within acceptable limits.
While some patients want noses that are quite upturned, too curved, obviously aesthetic, and shaped like hazelnuts, a group of patients prefer unobtrusive natural noses.
In women, the nose-lip angle is considered normal up to 105 degrees, but cases where it is forced up to 115 degrees are frequently encountered. There is a wide range of female nose types that are tried to be defined with terms such as Barbie nose, Hollywood nose, businesswoman nose, hazelnut nose, princess nose.
Before planning a female rhinoplasty, the patient’s expectations and the type of nose they want should be understood very well and the analyses should be done meticulously.
It is necessary to do not only nose analysis but also facial analysis. Aesthetic incompatibilities in areas such as the chin, cheeks, cheekbones, and forehead should be detected, and it is extremely important to suggest solution formulas.
Before and after rhinoplasty progresses differently in open and closed methods.
Before Rhinoplasty
A detailed examination before rhinoplasty and a clear understanding of the patient’s aesthetic expectations are the most important steps of the process. Depending on the content of the surgery, which techniques will be performed open or closed are determined at this stage in a way that is mutually agreed upon with the patient.
After Rhinoplasty
A night is spent under observation in the hospital after rhinoplasty. This also relaxes the patient in terms of getting through the first night without pain and comfortably. Ice application is applied correctly and monitored. The pain starts to subside from the second day onwards.
Depending on many parameters such as the technique used, the doctor’s experience, manual dexterity, and the type of rhinoplasty protocol applied, the fit of the nose after rhinoplasty and the change in rhinoplasty month by month show slight differences.
After a successful rhinoplasty surgery, if no complications develop, we can answer how the process progresses with the most frequently asked questions under the title of after rhinoplasty…
How Long Does Rhinoplasty Take?
Rhinoplasty is related to the difficulty of the case as well as the method and experience of the surgeon. Although the duration varies, it is approximately 2 hours in our practice.
Nose Tip Like a Ball After Rhinoplasty
We need to understand very well what kind of nose tip we envision before the surgery. Examples of pointed or round nose tips should be shown, the patient’s opinion should be asked, and the correct surgical method should be applied. However, it should not be forgotten that some patients may have supratip edema after surgery. In this case, it would be the right approach to wait patiently for the edema to pass.
Asymmetry in Nose Wings After Rhinoplasty
Asymmetry in the nose wings is a subject that should be examined carefully. The presence of this pathology should be noted in the preoperative analysis and photographs taken, and facial asymmetry should be investigated.
